The Anchorage Joint Sight Committee may be able to assist you with eye care if you meet the following criteria:
- No insurance of any kind (for example: no Medicaid, Denali Kidcare, VA, SouthCentral Foundation) available to obtain a prescription and/or glasses.
- Fill out the application completely and return to the Anchorage Area Lions Joint Sight Committee to be evaluated.

Complete the form and mail it to the address on the form. Your request for assistance will be considered at the next monthly Anchorage Joint Sight Committee meeting. If we need more assistance, we may contact you. You may also contact one of the agencies listed below.
