School Visit Instructions

Good day nurses and school staff,

You have asked that the Lions come in to help you with your student health screenings. We have put together a plan for what is expected from each of us when we come. Please give us a call should you have any questions or changes.

The Lions will arrive at your school 15 minutes before your testing start time, 15 minutes after the doors open. For elementary schools it is an 8:30 start time, middle schools 9:45 and high schools 9:00. The start times are one hour later on Mondays.

Please review the following items:

1 We will need to know how many students are to be screened as soon as possible, for each health screening type: Visionscreening, distance acuity and hearing. We will have to request the help of added volunteers if you have a larger group. In elementary schools we are usually done and out of your school by lunch. We do all 3 screenings types in elementary schools. For middle and high schools, we will have a morning and an afternoon shift to do both acuity and hearing testing during each period.

2 You have the choice of using our vision trailer or a room in the school for Visionscreening. Hearing and distance acuity must be done in the school. For in-school Visionscreening we will need to have a room where we are able to control the lights, preferably a room with no outside windows and a dimmer switch. If you would like us to use our trailer, please provide an aerial map of your school on which you indicate where we should park. We should be near an exit door and a power outlet. Use of the trailer is stopped after Thanksgiving. Inside your school we would like to have an adult chair for each of our Lions along with at least 2 long tables that are no more than 3 ft. deep, front to back to do Visionscreening. We do not need a chair for the students. They will stand in front of us to be screened unless they are very tall or very short. We will go to the Pre-K, K and Special Needs classrooms to do their screenings. For acuity testing we can do it in a hallway if no room is available, but we prefer a classroom or the library. Any area where we can set up 10-foot distance screening stations with good lighting would be acceptable. Hearing tests can be done in a single room where we can set up 4 single stations.

3 Please have the ASD Screening forms printed, front and back, for each student. The three forms you need have been set up by ASD Healthcare Services in Google Docs. The form is set up to print for each student group requested with student name, date of birth, grade and teacher listed. The age is needed because our results criteria for the Visionscreening are based on age.

4 The results label will have the child’s first name printed on it with the date to minimize the possibility of error. The completed forms will be returned to you at the end of the screenings for distribution to the parents.
5 Please have your own volunteers to lead the students to and from the classrooms. Our liability insurance will not allow us to be the escorts. We prefer to have the entire class brought down together. The screening process is very quick, and we will try to minimize the wait times between groups.

Please remember we are all volunteers. We will try to make this event as worry-free and as easy as possible for you and for us. We don’t want to disrupt your normal school day. Any questions? Please contact us.
